Pursuant to the “Law No. 6698 on the Protection of Personal Data” and “Regulation on the Processing of Personal Health Data and Ensuring Privacy”, I have been trained within the company in terms of the relevant legal regulations and the rights I have within this framework, and I have read and reviewed the clarification text delivered to me after the training. Within the scope of the relevant Laws, my necessary personal data, special categories of personal data, health data, including those that identify or serve to identify my identity, given to SMARTES TEKNOLOJİ YAZILIM DANIŞMANLIK SAN.
